Introduction

SDA Services would like to find out how living in Specialist Disability Accommodation (SDA) has made an impact on people’s lives.

We would like people who have lived in SDA for 6 months or more to complete a survey. The survey asks about people’s lives and supports before and after living in SDA.

After completing the survey you can enter a prize draw to win one of 5 x $100 Freedom vouchers.

How can I complete the survey?

The survey will take around 15 minutes depending on your answers. It might help to have your NDIS plan with you to answer some questions about your SDA type and supports.

You can do the survey by clicking the link below. If you think you will need a break you can save the survey, copy your URL and paste it or email it to yourself.

You can ask someone you trust to help you. This might be helpful for some questions that ask for details around your support.

You can also call SDA Services, if you need phone assistance or would like them to speak to someone on your behalf.

What happens to my answers?

The information will help to understand any changes in people’s life from using SDA. SDA Services may provide results to the sector and on their website. No individual will be identified.

Personal information will remain private. That means:

• What you tell us is not given to other people, unless we are required to do so by the law or you give us permission.
• We will only use the information you give for the purpose of conducting this research, and in line with the Privacy Laws.
